Temporary Nurse - Assistant to the Clinical Care Coordinator

SUMMARY:  As a temporary nurse assisting the Health and Well-Being team, works with Clinical Care Coordinator in nursing care management to SNJMs living in California.  Consults and advises regarding healthcare needs and services.   Performs all essential functions of care coordination.  Under supervision of Clinical Care Coordinator, coordinates daily support services as needed by individual Sisters. Enhances each Sister’s understanding of her own medical needs.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
 
  1. Assists and advises individual Sisters who have requested assistance or who have been identified as needing assistance with healthcare and related services.
 
  1. Performs all essential functions of care coordination which includes assessment, development of health and related needs-based plan, implementation, monitoring, evaluation and adjustment.
 
  1. Assists Clinical Care Coordinator with supervising and coordinating daily support services provided by SNJM Health & Well-Being staff and agencies staff. 
 
  1. Provides direct nursing care services and daily support services with the approval of the Clinical Care Coordinator.
 
  1. With Clinical Care Coordinator works closely with the SNJM California Benefits Coordinator in areas relating to insurance, benefits, and special orders regarding equipment, medications and medical supplies.
 
  1. As part of the California SNJM Health & Well-Being team:
    1. May attend weekly SNJM care team meeting 
    2. May attend care conferences at facilities
    3. Serves as nursing resource to SNJM Community Life Coordinators
 
  1. Maintains positive, professional working relationships with staff at Merrill Gardens, The Villa at Saratoga and other facilities to insure optimal care of Sisters.
 
  1. Assisting Clinical Care Coordinator with coordination/use of healthcare providers (physicians, discharge planners, home health agencies, hospice, etc.) as appropriate.
 
  1. Transports and accompanies Sisters to medical appointments when health advocacy is needed, or supports others in doing so, especially at times of health transitions, surgery, etc.
 
  1. Assists in transitioning Sisters as needed to other levels of care. 
 
  1. Available upon request to assist Sisters not living in facilities with their healthcare areas.
 
  1. Serves as a resource to Sisters and their chosen support person(s) especially during times of health transitions, serious medical conditions and/or surgeries.
 
  1. Provides and/or assists with education of Sisters about health and wellness topics.
 
  1. May occasionally be requested to be available for consultation on off hours when Clinical Care Coordinator is not available. 
 
  1. Other related duties as needed and/or directed.
 
 
INTERRELATIONSHIPS:  Works in close collaboration with Sisters, Province Health and Well-Being Team, Merrill Gardens, The Villa at Saratoga, and Mercy Care and Retirement Center, SNJM Community Life Coordinators, California H & WB Team members, and others as necessary.
 
QUALIFICATIONSTo perform this job successfully individuals must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skills, and/or ability required.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
 
Education:                 Graduation from an accredited LVN or RN nursing program.
Experience:               LVN or RN with 5 – 8 years of working with the geriatric population in various settings.

  6. Understanding of and ability to maintain appropriate professional boundaries.
  7. Demonstrated knowledge of and experience coordinating community resources in South Bay area and ability to research, evaluate, and coordinate community resources in other areas of California as needed.
  8. Ability to coach, direct and lead staff in collaborative and respectful manner.  Knowledge and ability to supervise in-home care.  Ability to develop and maintain collaborative relationships with SNJM Sisters, staff, and external contacts.
  9. Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with Sisters, staff, providers, and external facilities.
  10. Excellent organizational skills and ability to prioritize and execute tasks in a timely manner.
  11. Demonstrated written and verbal skills to assure effective communication.
  12. Ability to respond to urgent and/or stressful situations appropriately and calmly.
  13. Proficient knowledge of Microsoft Office and other computer software programs including, but not limited to Word, Excel, Outlook.
  14. Valid drivers' license and personal vehicle  
  15. Ability to travel frequently within South Bay/Santa Cruz area in carrying out job responsibilities. Occasional travel in other California locations as needed.
 
 
HOURS WORKED: Variable, 32 to 40 per week  
 
PHYSICAL DEMANDS AND/OR WORK ENVIRONMENT:
Position requires work in office and residential settings, along with travel to conduct site visits to Sisters’ homes and/or medical facilities.  Must be able to assist Sisters with transportation, and with getting into and out of vehicles. Requires ability to lift/maneuver wheelchairs, walkers, and other medical equipment.  Be able to lift 20 lbs.
 
Occasional evening and/or weekend hours as needed. 
 
EQUIPMENT USED:
Computer (PC), keyboard, mouse; office telephone/voice mail system; fax/copier, scanner, etc.  .
