Clinical Care Coordinator

About This Role: As part of the Province Health and Well-Being department, this position provides support to the Sisters of the Holy Names of Jesus and Mary (SNJM) residing in the Seattle, Washington area. The RN consults and advises regarding healthcare needs and services, performing all essential functions of care coordination. Under the supervision of the Director of Clinical Care, they will coordinate daily support services as needed by individual Sisters and helps to enhance each Sister’s understanding of her own medical needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Assists and advises individual Sisters who have requested assistance or who have been identified as needing assistance with healthcare and related services to support their independence and individual preferences.

2. Performs all essential functions of care coordination which includes assessment, development of health and related needs-based plan, implementation, monitoring, evaluation, and adjustment.

3. Transports Sisters to medical appointments when needed, and provides advocacy in the current healthcare environment, including accompaniment requested by a Sister for important appointments, during health transitions, for medical testing, surgery, etc.

4. Transports/arranges transport/or meets Sister at the Emergency Department. Assists in care coordination and discharge planning.

5. Assists Director of Clinical Care with coordination/use of healthcare providers (physicians, discharge planners, home health agencies, hospice, etc.) as appropriate.

6. Assists in transitioning Sisters as needed to other levels of care.

7. Serves as a resource to Sisters and their chosen support person(s) especially during times of health transitions, serious medical conditions and/or surgeries.

8. Provides direct nursing care services and daily support services with the approval of the Director of Clinical Care.

9. Keeps Director of Clinical Care (and DPOA’s as appropriate) involved in a particular Sister’s health status, updated on a regular basis. Sends weekly summary to Washington Director of Clinical Care.

10. Works closely with the SNJM Assistant Director of Healthcare Services Washington Benefits Coordinator in areas relating to insurance, benefits, and special orders regarding equipment, medications, and medical supplies.

11. Documents pertinent clinical information and updates in the SNJM database in a timely manner.

12. Maintains positive, professional working relationships with staff at care facilities to ensure optimal care of Sisters.

13. Provides and/or assists with education of Sisters about health and wellness topics.

14. Directly supervises Seattle-area Caregiver.

QUALIFICATIONSTo perform this job successfully individuals must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or ability required. Education: Graduation from an accredited nursing program; BSN preferred

Experience: RN with 5 – 8 years of working with the geriatric population in various settings.

Certification: RN license-State of Washington

HOURS: 24-30 hours/week. Occasional evening and/or weekend hours as needed. Rare overnight travel may be necessary for training/conferences.

PAY RATE: $52/hour

  • Ability to respect and maintain confidentiality of sensitive information and to exhibit good judgment in decision making.
  • Thorough knowledge and understanding of the concepts of aging and concerns of the elderly population.
  • Demonstrated ability to communicate professionally, empathetically, and compassionately with elder population who may be ill, disabled, hospitalized, and/or experiencing loss.
  • Understanding of and ability to maintain appropriate professional boundaries.
  • Demonstrated knowledge of and experience coordinating community resources in the Seattle area.
  • Knowledge and ability to supervise in-home care.
  • Must be able to function independently and have flexibility, personal integrity, and the ability to develop relationships and work collaboratively with Sisters, staff, providers, and external contacts at facilities.
  • Excellent organizational skills and ability to prioritize and execute tasks in a timely manner.
  • Demonstrated written and verbal skills to ensure effective communication.
  • Ability to respond to urgent and/or stressful situations appropriately and calmly.
  • Proficient knowledge of Microsoft Office and other computer software programs including, but not limited to Word, Excel, Outlook.
  • Valid drivers' license and personal vehicle; good driving record.
  • Ability to travel frequently within the Seattle area to fulfill job responsibilities.
